Why Patient-Specific Manufacturing Is Becoming the Next Frontier in Radiation Oncology

Radiation oncology has spent two decades getting better at aiming. Image guidance, intensity modulation, stereotactic delivery and now adaptive replanning have all pushed toward the same objective: put dose where the tumor is and nowhere else. The gains have been real and well documented.

But the field is approaching a limit that better beam shaping cannot solve. Normal tissue that sits inside or immediately adjacent to the target volume receives dose because it is physically there. No amount of modulation moves it. That is a geometry problem and geometry problems are solved with hardware.

Which is why the next meaningful advance in this field may not come from the linear accelerator at all. It may come from the accessories that sit against the patient and specifically from the ability to manufacture those accessories for one person, one anatomy and one treatment plan at a time.

The accessory problem hiding in plain sight

Consider head and neck cancer. For patients with oral cavity and oropharyngeal disease, intraoral devices have been used for decades to open the mouth, displace the tongue or palate away from the high-dose region and reproduce the same position across thirty-odd fractions. The dosimetric rationale is not controversial. Displacing healthy tissue out of the field reduces the dose it absorbs.

What is remarkable is how these devices are still made. In most centers, they are fabricated by hand, chairside, using dental impression materials and the skill of whoever is available that day. The result is often clinically adequate. It is also variable between patients, variable between operators, poorly documented and expensive in the one resource cancer centers have least of, which is clinical staff time.

No one would design a modern medical device this way. The field tolerates it because, until recently, the alternative did not exist at a workable cost.

Why this specialty is the natural proving ground

Three structural features make radiation oncology unusually ready for patient-specific manufacturing.

First, the digital anatomical model already exists. Every patient receives a simulation CT as standard of care. The imaging burden of designing a custom device is therefore zero, which is not true in most other specialties.

Second, the devices themselves are geometrically demanding but functionally simple. No electronics, no power source, no drug, no permanent implantation and short-duration contact with intact tissue. That risk profile fits comfortably within a 510(k) pathway, which means development timelines measured in quarters rather than years.
